What precautions are necessary when using atropine in older populations?

Preparing a safety protocol for my unit, I want to move from vague "use with caution" guidelines to actionable, mandatory steps. I'm interested in the non-obvious checks beyond just lowering the dose that prevent adverse events and hospital readmissions in frail patients.

My mandatory checklist includes a pre-administration screen for undiagnosed narrow-angle glaucoma (ask about halo vision) and for prodromal BPH symptoms. I always check for pre-existing, often silent, cardiac arrhythmias via a recent ECG. Post-administration, I recommend scheduled checks for bowel sounds and urinary output over the next 24 hours, not just vitals. I've seen constipation or retention trigger downward spirals. Have a low threshold to involve a geriatric specialist.
