How do I format the title page for a PhD synopsis submission?

I'm preparing the title page. What information is mandatory? Should it include keywords, the department name, the degree name (Ph.D.), and the submission date? Is there a standard centered layout?

A professional title page is non-negotiable. It typically follows a centered, hierarchical layout and must include:

  1. Full Title of the Research (in a large, bold font)
  2. "A Synopsis Submitted for the Degree of Doctor of Philosophy (Ph.D.)" or similar phrasing
  3. Your Full Name
  4. Department/School Name
  5. University Name
  6. Name(s) of Supervisor(s) ("Under the guidance of...")
  7. Month and Year of Submission
