When preparing a Questionnaire with both quantitative and qualitative methods, divide the questionnaire into multiple sections and each section will be having specific questions. For Example: Section 1 _ Demographic variables  Section 2 _ Likert Scales questions on Main Variables Section 3_ Open Ended questions   Since there are 2 methods of approach, it's better to have fewer qualitative questions if the quantitative questions are more.
