Points to keep in mind while making a questionnaire?

Dissertation

I have tried making the questionnaire many times, but my supervisor has always rejected it. This is my last chance if, this time also he rejects it then my thesis would be ruined. He won’t accept or give me any chance further.
Please tell me the key points to keep in mind while making a questionnaire.
