Designing interview questions for a qualitative research

I need assistance in questionnaire designing for my qualitative research on the people suffering from obsessive–compulsive disorder (OCD). I have prepared the draft of the questionnaire with help of my colleague but that has been rejected by advisor as he believes that the questionnaire doesn’t follow ethical considerations laid out by the university in terms of language as well as the issues that I’m exploring. So I’m feeling the need of a professional or expert at questionnaire designing. If anyone is here, please share your contact details…I really need help!
